Background to Personal Information Protection at Hitachi High-Technologies

The following replies to requests for the disclosure, etc, of personal information other than requests for the disclosure, etc, personal information other than requests for the disclosure of personal information provided by the customer to a business department at HHT or requests for the disclosure, etc, personal information submitted via an HHT website.

1.Request Format

Please download the Request form for Disclosure,etc, of Personal Information (PDF format). If the customer is unable to download the Request form for Disclosure,etc, of Personal Information (PDF format), HHT will forward the form by fax or by mail. (Please send us an inquiry using the Inquiry Form.)

2.Attachments

Please attach the following two documents to the Request form for Disclosure,etc, of Personal Information (PDF format).

a)	ID documents for the person concerned (For details, please refer to About the Customer's ID Documents.)
b)	Proof of fee payment (Not necessary in case of the correction, addition or deletion of personal information)

3.Payment of Fee

A fee of 800 yen is payable for disclosure of personal information and notification of the purpose of use. For details, please refer to About Methods of Fee Payment. No fee is payable in case of correction, addition, deletion, stopping usage or stopping submission of personal information to third parties.
Further, the fee will not be returned if there is no disclosure or notification of purpose of use due to reasons set out in laws and ordinances, or if HHT does not hold personal information on the customer.

4.Request Methods

Please send the documents listed in a) to c) below in a sealed envelope by a recorded method of delivery such as registered mail to the Contact Desk below 5. (Cost of postage to be borne by the customer.)

a)	The completed and signed Request form for Disclosure,etc, of Personal Information(PDF format).
b)	ID documents for the person concerned (For details, please refer to About the Customer's ID Documents.)
c)	Proof of payment of fees (For details, please refer to About Methods of Fee Payment.)

6. Response from HHT

The reply to the customer will be sent by registered mail for delivery to the named person (special delivery) at the address recorded on the ID document. With regard to the response from HHT, please allow an interval of two weeks for the internal inquiry. If more than two weeks are required, HHT will make contact to that effect.
